Description Since 1987, Homes for Life Foundation (HFLF) has established a network of permanent, affordable, service-enriched housing in Los Angeles County for individuals who suffer from persistent mental illness and homelessness. HFLF is seeking a Mental Health Clinician (ACSW or AMFT) to manage a caseload of up to twelve residents living in state-licensed Adult Residential Facilities (ARFs) located in Torrance, California. This is a full-time position, Monday - Friday, 9am - 5:30pm, with on-call responsibilities for crisis response including evenings, weekends, and holidays. Clinical supervision provided!
DUTIES and RESPONSIBILITIES (not all-inclusive) Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
• Provide individual and group sessions; deliver and document mental health rehabilitation and targeted case management services in accordance with agency and Los Angeles County Department of Mental Health (DMH) standards; monitor, track, and complete Medi-Cal charting in Welligent, an Electronic Health Records System (EHRS) • Design treatment plans and link residents to community resources; develop on and off-site support programs and activities including individual / group sessions, community outings, house meetings, volunteer / vocational opportunities, and other programs aimed to improve residents’ quality of life, self-esteem, and stability • Assist with screening and selection of residents; work with all program staff to ensure all service needs are met; perform outreach as needed • Perform advocacy; assist residents in securing appropriate services and maintaining income and benefits • Act as liaison between residents and their family members, guardians, Conservators, etc. as needed • Complete person-centered Appraisal Needs and Services Plans, Incident Reports, and other Community Care Licensing paperwork as needed • Provide client-related support to residential staff • Attend trainings and meetings as may be required by HFLF; collaborate with DMH, mental health agencies, and other external / internal entities as needed; communicate regularly with HFLF’s Executive Director / Facility Administrator on client and/or program-related issues • Regularly transport residents in personal vehicle
EDUCATION / EXPERIENCE / SKILL REQUIREMENTS
• Requires Master’s degree and active registration with the CA BBS [i.e., ACSW, AMFT] • Two years’ relevant experience, including DMH documentation and residential experience preferred • Knowledge of community and educational resources, with ability to provide independent living skills training • Excellent administrative and computer skills to allow for the timely completion of accurate progress notes and other client record maintenance; experience with Welligent or other EHRS a plus; proven track record of successfully meeting deadline-driven productivity requirements • Excellent interpersonal skills, with the ability to build professional and effective rapport with residents and their family members, program staff, external partners and service providers, upper management, etc.; displays sensitivity, empathy, and compassion at all times • Excellent initiative and follow-through with ability to manage workload with minimal supervision and/or without prompts • Excellent communication skills required (verbal and written) • Utilizes a safe and reliable vehicle; requires excellent driving skills to safely transport residents

Skills 2+ years' experience with crisis response, including calling PET/PMRT, highly desirable Subsequent to conditional job offers and prior to beginning employment, candidates must provide Live Scan and Health Screening/TB clearances, First Aid certification, valid CA Driver’s License, safe driving record from DMV, and proof of adequate vehicle insurance, including Uninsured (UI) Motorist coverage; other requirements may apply at time of hire
