Position Summary:
The Licensed Vocational Nurse (LVN) provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN) or Clinical Supervisor. The LVN ensures care is delivered in accordance with physician’s orders, the established plan of care, and applicable state/federal regulations.
Key Responsibilities:

Deliver skilled nursing care including wound care, medication administration, catheter care, and monitoring vital signs.

Observe and report changes in patient condition to the RN and/or physician.

Assist in implementing patient-specific care plans and interventions.

Document patient care, observations, and services in electronic medical records (EMR) promptly and accurately.

Educate patients and caregivers on disease processes, medications, and self-care techniques.

Participate in case conferences, team meetings, and ongoing staff education/training.

Comply with HIPAA regulations and all clinical policies, procedures, and standards.

Provide emergency intervention as appropriate and report incidents promptly.

Communicate effectively with interdisciplinary team members and maintain professional relationships with patients and families.
Qualifications:

Current and valid California LVN license.

Minimum of one (1) year of experience as an LVN, preferably in home health or acute care.

Current CPR/BLS certification.

Valid California driver’s license and reliable transportation.

Proof of current auto insurance.

Proficiency in using electronic medical record systems (experience with [insert EMR system if known] is a plus).

Strong interpersonal and communication skills.

Ability to work independently and with a team in a fast-paced environment.
Preferred Qualifications:

Experience working with geriatric or chronically ill populations.

Bilingual (Spanish, Tagalog, etc.) preferred but not required.

IV certification (if applicable to services provided).
