Coordinated Family Support Staff (CFS)

Description: The Coordinated Family Support (CFS) Staff provides person-centered services to adults with developmental disabilities who reside in the family home. This role supports individuals and their families by helping them navigate the service system, coordinating resources, providing education and training, and ensuring the individual’s needs and goals are met to maintain community-based living and independence.

Minimum Experience: 3 years, providing services to adults with developmental disabilities

Work Hours: Part-time, hours may vary per week, based on case load

Job Requirements: Reliable transportation and ability to cover a 15 to 20-mile coverage area, pass a criminal background check, current physical, and TB test.
