What you bring to SBHG:
- Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience)
OR
- Bachelor’s degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)
- Valid California Driver’s License required
How you will make a difference:
- Provides services at the office, in the community, and in clients’ homes. Uses a personal vehicle to drive within the community to provide services and to transport clients in the personal vehicle.
- Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
- Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through.
- Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars. Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
- Monitors the clients’ progress toward goals outlined in their treatment plans. Informs the primary therapist of progress.
