What you bring to SBHG:

  • (4) years of experience in a social service field OR
  • Associate Degree plus Two (2) years of experience in a social service field OR
  • Bachelor’s degree in a social science field plus at least Six (6) months experience in a social service field or a school internship in a social service field
  • Valid California Driver's License.
  • Bilingual in Spanish preferred

How you will make a difference:

  • Provides services at the TAY Center, in the community, and in TAY’s homes.
  • Uses personal vehicle to drive within the community to provide services and to transport TAY and/or TAY’s family members in personal vehicle.
  • Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
  • Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through.
  • Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Stars.
  • Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
