Position Summary:
The Speech Therapist (SLP) is responsible for providing diagnostic evaluations and therapeutic interventions to patients with speech, language, voice, and swallowing disorders. Services are delivered in the patient's home, and the SLP collaborates with the interdisciplinary team to help patients improve communication and quality of life. This position requires routine travel to patient residences within the service area.
Key Responsibilities:

Travel to patients' homes to evaluate and treat individuals with speech, language, cognitive-communication, voice, and swallowing disorders.

Develop individualized care plans based on comprehensive assessments and physician orders.

Provide direct therapy using evidence-based techniques and interventions.

Educate patients and families on communication strategies and at-home exercises.

Document all care and communication in the electronic medical record (EMR) accurately and timely.

Collaborate with physicians, nurses, occupational and physical therapists, and other care team members.

Re-evaluate and adjust care plans based on patient progress and needs.

Ensure compliance with all Medicare, federal, state, and agency regulations and standards.
Qualifications:

Master’s degree in Speech-Language Pathology from an accredited institution.

Current and valid California Speech-Language Pathologist license.

Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA).

Minimum of one (1) year experience as a licensed SLP (home health experience preferred).

Current CPR/BLS certification.

Valid California driver’s license, reliable transportation, and active auto insurance.

Willingness and ability to travel to patient homes daily.

Strong documentation and communication skills.
Preferred Qualifications:

Experience working with adult and geriatric populations.

Knowledge of dysphagia treatment and alternative communication methods.

Bilingual (Spanish, Tagalog, etc.) preferred.

Familiarity with home health EMR platforms.
