Position Summary:
The Physical Therapist (PT) provides skilled therapy services to patients in their homes, promoting mobility, function, and independence through individualized care plans. The PT evaluates, treats, and educates patients and their families, ensuring safe recovery and maximum functional outcomes in accordance with physician orders and home health regulations. This position requires regular travel to patient residences within the assigned service area.
Key Responsibilities:

Travel to patients' homes to conduct initial and ongoing physical therapy assessments and develop individualized treatment plans.

Provide therapeutic interventions including exercise programs, gait training, balance activities, and pain management.

Educate patients and caregivers on home exercise programs, equipment use, and safety strategies.

Collaborate with the interdisciplinary team, including RNs, OTs, and physicians, to coordinate care and adjust treatment plans as needed.

Monitor and document patient progress, treatment responses, and changes in condition.

Recommend and instruct in the use of assistive devices, mobility aids, and environmental modifications.

Comply with all federal, state, and agency regulations, including Medicare guidelines.

Participate in team meetings, case conferences, and staff training.

Maintain accurate and timely clinical documentation in the electronic medical records (EMR) system.
Qualifications:

Current and valid California Physical Therapist license.

Graduate of an accredited Physical Therapy program.

Minimum of one (1) year of physical therapy experience (home health preferred).

Current CPR/BLS certification.

Valid California driver’s license, reliable vehicle, and active auto insurance.

Willingness and ability to travel daily to multiple patient homes within the service area.

Proficiency in documenting via EMR systems.

Strong interpersonal, clinical, and communication skills.
Preferred Qualifications:

Experience working with elderly and medically fragile populations.

Bilingual (Spanish, Tagalog, etc.) a plus.

Knowledge of Medicare home health regulations.
