Position Summary:

The Occupational Therapist (OT) provides skilled therapy services to clients in their homes to enhance their ability to perform daily activities safely and independently. The OT evaluates, plans, and implements treatment to help patients restore function, compensate for limitations, and adapt their home environments.
This position requires routine travel to patients’ homes throughout the assigned service area.


Key Responsibilities:

  • Travel to patient homes to evaluate functional abilities and environmental challenges.
  • Develop individualized treatment plans focused on improving ADLs (activities of daily living), fine motor skills, and safety.
  • Educate patients and caregivers on adaptive techniques, home safety, and use of assistive devices.
  • Recommend equipment modifications and home adaptations to increase independence and reduce risk.
  • Collaborate with the interdisciplinary care team to coordinate and update patient care plans.
  • Monitor, document, and report patient progress using the electronic medical record (EMR) system.
  • Provide training and supervision to COTAs (Certified Occupational Therapy Assistants) as applicable.
  • Participate in patient case conferences, staff meetings, and continuing education as required.
  • Ensure compliance with state and federal guidelines, including Medicare home health regulations.

Qualifications:

  • Current and valid California Occupational Therapist license.
  • Graduate of an accredited Occupational Therapy program.
  • Minimum of one (1) year of experience as an OT (home health preferred).
  • Current CPR/BLS certification.
  • Valid California driver’s license, reliable transportation, and current auto insurance.
  • Comfortable with regular travel to patient residences.
  • Strong assessment, planning, and communication skills.
  • Familiarity with EMR systems and documentation best practices.

Preferred Qualifications:

  • Experience working with seniors and physically or cognitively impaired individuals.
  • Bilingual (Spanish, Tagalog, etc.) is a plus.
  • Knowledge of home modification strategies and durable medical equipment.
