Monday - Friday business hours

What you bring to SBHG:
- Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience)
OR
- Bachelor’s degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)
- Valid California Driver’s License required.
How you will make a difference:
- Conduct a housing screening on all participants involved in the FSP Program.
- Develop an intervention strategy based on the needs of the client.
- Incorporates the Transition to Independence Process (TIP) into services.
- Advises Peer Specialist/Family Advocate, Mental Health Specialists, and Personal Service Coordinators on housing and resource options for families and participants.
- Collaborates with program staff to ensure housing, education/employment, and resource needs are met.
- Attends Team meetings, as needed.
- Documents all services provided clearly and concisely within the Electronic Health Record within the timelines and framework established by Central Star.
- Coordinates community contacts for events and services to be brought into the program. Develops and maintains a list of available resources accessible to all team members and families.
- Participates in all outreach functions, community meetings, and family meetings as assigned.
- Intervenes with the client in an emergency crisis to provide support and assistance in problem resolution.
