22.00 - 28.00 USD Per Hour

Working Location:

Community-based in Fresno County

Working Hours/Shift:

Monday - Friday business hours

What you bring to SBHG: 

  • Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience)

OR

  • Bachelor’s degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)
  • Valid California Driver’s License required.

How you will make a difference: 

  • Conduct a housing screening on all participants involved in the FSP Program.
  • Develop an intervention strategy based on the needs of the client.
  • Incorporates the Transition to Independence Process (TIP) into services.
  • Advises Peer Specialist/Family Advocate, Mental Health Specialists, and Personal Service Coordinators on housing and resource options for families and participants.
  • Collaborates with program staff to ensure housing, education/employment, and resource needs are met.
  • Attends Team meetings, as needed.
  • Documents all services provided clearly and concisely within the Electronic Health Record within the timelines and framework established by Central Star.
  • Coordinates community contacts for events and services to be brought into the program. Develops and maintains a list of available resources accessible to all team members and families.
  • Participates in all outreach functions, community meetings, and family meetings as assigned.
  • Intervenes with the client in an emergency crisis to provide support and assistance in problem resolution.
