The Position: The STRTP Facilitator provides community inclusion supports by working with a child or adolescent within the scope of facilitating CFTMs, coordinating pre meetings, obtaining necessary information from authorized representatives, clients and treatment team members.
Compensation and Benefits:
The pay range we’re offering is $25-30/HR, depending on experience.

Qualifications for Employment
- Master's degree in a behavioral science from an accredited college or university, plus two years of employment as a social worker, as defined in California Code of Regulations, Title 22, Section 80001 (s)(4), in an agency serving children or in a residential congregate care program for children; or
- A Bachelor’s degree in social services and 4 years of experience in a mental health setting as a specialist in the fields of physical restoration, social adjustment or vocational adjustment; or
- Up to 2 years of graduate professional education may be substituted for the experience requirement on a year to year basis; or
- Up to 2 years of post associate arts clinical experience may be substituted for the required educational experience in addition to the requirement of four years’ experience in a mental health setting (source: Title 9, Division 1, Section 630).
- Experience serving individuals with complex needs including Severely Emotionally Disturbed (SED) youth/ and families and/or youth in residential settings required.
- Experience with the mental health, child welfare and/or juvenile probation systems required.
Responsibilities:
- Provides trauma informed team facilitation to support and enhance the efforts of youth and their families/caregivers, system workers who are part of the support team, and other individuals in a variety of environments.
- Ensure that the principles of the Core Practice Model are adhered to by all team members, including the identification of client and family strengths, the providing of services in a timely and appropriate manner, and being the primary contact point for children, families, service providers, and community resource personnel.
- Act as liaison between children within the STRTP, family members/identified supports, service providers, DCFS personnel, and the community for the purposes of providing case management, advocacy, needed resource information and linkage, and highly coordinated services as directed by the Child and Family Team (CFT).
- Provides facilitation, assessment, and case management services including all related documentation for services planned and provided.
- Collaborate with all CFTM members.
- Complete documentation of interactions within a 24 hour period.
- Coordinate CFTM meetings, Emergency CFTMs, Preservation CFTMs and Staffing’s for the youth In the STRTP program.
- Meet service delivery expectations.
- Attend supervision weekly with the supervisors.
- Attend all STRTP related training as required by the program and agency.
- Other duties as assigned.
