Position Summary:
The CNA/HHA provides non-medical personal care and assistance with activities of daily living (ADLs) under the supervision of a Registered Nurse. This role plays a vital part in supporting patients’ health, safety, and independence at home.
This position requires daily travel to patient residences within the agency’s service area.
Key Responsibilities:

Travel to clients’ homes to provide personal care such as bathing, grooming, dressing, toileting, and feeding assistance.

Assist patients with mobility, transfers, and safe ambulation.

Perform light housekeeping tasks related to patient care (e.g., changing linens, washing dishes, tidying patient areas).

Take and record vital signs (temperature, pulse, respiration, and blood pressure) as instructed.

Observe and report changes in patient condition to the supervising nurse or case manager.

Support patients with prescribed exercises and range-of-motion activities as directed.

Offer companionship and emotional support to patients and families.

Assist with reminders for medication (without administering medications).

Maintain accurate records of care provided using agency documentation tools or EMR.

Adhere to infection control and safety procedures during all visits.

Follow the individualized care plan and instructions provided by clinical supervisors.
Qualifications:

Current California CNA certification.
-
Current California Home Health Aide (HHA) certification.

Current CPR/BLS certification.
-
Minimum of six (6) months experience in home health, hospice, or personal care (preferred).

Valid California driver’s license, reliable transportation, and current auto insurance.

Ability and willingness to travel daily to multiple home visits.

Strong interpersonal and communication skills.

Ability to follow instructions and document care clearly.
Preferred Qualifications:

Experience working with elderly, chronically ill, or developmentally disabled individuals.

Bilingual (Spanish, Tagalog, etc.) is a plus.

Familiarity with basic EMR systems.
